Bruh I saw the episode today. It’s very subtle but he receive the scar while jimbei is holding him in the air, and he gets pierced by akainu. Jimbe gets a huge hole in belly, while luffy gets the only a surface cut. They even explain this while Law is taking care of them in the submarine.
