Demon's Souls isn't a land of fog, the Old One appearing brings the demon fog which is covering several kingdoms in fog which threatens to keep spreading.

My favorite connected universe theory is that demon's souls gets consumed by fog, becoming the land of Dark Souls, the Great Old One having consumed all the souls, then it caught a spark, and ignited into the first flame, lastly in DS3 the painter paints a new world, one painted in blood, free of the restraints of Fire, which becomes Bloodborne

Do also keep in mind that vaddividya also had a theory about how multiple endings go in souls games. No matter which ending you choose the next game still happens. For example in dark souls one you can either choose to link the flame or let it die. However no matter which choice you choose dark souls 2 will still end up happening.
