Pump Jack explosion was due to using a steel pipe wrench with a steel hammer causing a spark triggering the gas leak coming from the loose hammer union, the crew neglected to use a sparkless hammer and an aluminum wrench, could have also just used a bigger size wrench and a cheater pipe/bar, common sense basic oil field 101

I work in oil and gas and had something similar happen in my company, a 24 year old with a newborn died and 3 were injured in the blast.

If you want to be completely honest it's that old saying it's a little bit of everyone's fault. 1. The pusher of the crew using a (steel)hammer with a (steel) pipe wrench instead of using a brass hammer to not create spark, or just use a "cheater pipe" on the wrench for more leverage to open valve slowly. Also him being the boss of the crew should have done a step by step safety walk through checking for leaks (example: Check the hammer union that was loose). 2. The crew after the workover rig(pulling unit) not tightening or not checking all piping that they may have replaced or removed. 3. The actual oil company not keeping their equipment and piping checked regularly via (pumper or production operator). At the end of the day 3 people lost their lives due to poor communication within the company and faulty equipment. Just remember alot of the times someone will find another to point the finger of blame, instead of taking responsibility.

the pumper and his crew violated about a dozen safety protocols that would have never been allowed to happen in the field As a manager I got reamed out by executive mgmt for a helper cutting his hand from an exposed bolt on equipment that he was is the real oil and gas world

Stupidly using a metal wrench as a hammer causing sparks to form and poorly maintained pump jack. If you use a hammer it should be non-sparking brass. Pipe wrenches should be aluminum with non-sparking teeth. Simple safety. Metal wrenches are used by cheap asses who don't care bout safety.
