It will take a year or two but the USCSB channel will make a video about how this happened and how it could have been prevented.

Any steam operator will well know what it was. BLVE! Boiling liquid vapor explosion. Those are basically large pressure cookers(boilers that operate under considerable pressure) Their required to be equipped with safety pressure relief valves to prevent this result. Basic laws of liquid apply. Water, when turned into steam the volume expands 1700%. In a confined pressure vessel that will produce considerable raise in pressure. As the pressure rises, it also raises the boiling point considerably, and the temp in the liquid. If the vessel ruptures, all the still liquid inside instantly converts to vapor and that unchecked expansion (BOOM) Is your explosion. The released force is like a large bomb detonated. Those cookers aren't real heavy built so it don't take too much over pressure to rupture one. So in all likely hood the workers aren't trained in the dangers of any failures of the safety equipment. It all gets back to lack of plant management.

It was a money error... they (The company in question) didn't want to spend the money.

As so with many incidents like this or similar within large companies and corporations....follow the money.

The headline says the cause of explosion was released. Yet the video states no cause, merely says that vessel 6 exploded.

The safety regulations removed back in 2016 might have helped, but who needs those things anyway. Right? Just like trains don't need safety regs, neither do factories.

FYI There was another explosion that happened 20 years ago at that same building

You know, there's a reason for routine inspection of pressure vessels and their pressure relief systems. Sad for the loss of jobs, housing, and injuries.

According to some, the math skills and attention to detail required to safely run processes like this sets an unfair benchmark.

THERE'S NEWS: The tank that exploded was not vented properly. How much education was required to determine that?

There is nothing that can be done now to bring back the dead and heal the injured. The question is, will we enforce safety regulations and requirements of let companies do whatever they want with impunity?

All comes down to inspwction and maintenance, basically it happened because it was cheaper than preventing it from happening.

Of course it was human error. There is always a chain of events leading to a disaster.

What agency has jurisdiction over these large pressure vessels? USCSB?

The usual lazy, greedy and incompetent suspects.

They'll find someone to blame.
