Answer 1::
Recently the government of India notified a new policy on rare diseases called the national policy for rare diseases 2021
The policy is based on the principle of inclusion

Some of the salient features include:
(1) one time treatment funding cost of up to RS. 20 lakhs
(2) crowd funding mechanism
(3) creation of a registry of rare diseases
(4) provisions for early detection

The policy defined rare diseases as those that in frequently occur in population and other parameters
That is total number of people with the disease, its prevalence and the availability or non - availability of treatment options

The WHO defines rare disease as having a frequently of less than 6.5 to 10 per 10, 000 people
it is Estimated that there are about 7000 Known cases of diseases with an estimated 300 million people suffering of which 70 million are in India
Some of the rare diseases include inherited cancers, autoimmune disorders, cystic fibrosis muscular dystrophies and lysosomal storage disorders

Analysis of the policy:
(1) the policy concentrated on factor such as defining diseases, providing a scope for development and commercialisation of drugs for treatment and improving funding
(2) it doesn't provide a complete solution for the immediate and lifelong treatment needs and for therapies approved by the drug controller general of India
(3) the cost to help already diagnosed patients might be in the range of RS. 80 to 100 controller general of India
(4) these would be burdensome either for the states or the centre to bear them independently
(5) so, to reduce the costs, the government of India can have already been worked out with states like Kerela, Tamil Nadu and Karnataka with other states to reduce the burden

Conclusion:
Thus, the policy is a step in the right direction for securing the well - being of everyone irrespective of than constituting critical mass or not

Q.1 Critically evaluate the recent notification of the National Policy for Rare Diseases 2021. (250 words; 15 marks)

ANS.1) National Policy for Rare Diseases 2021:- It offers financial support and introduces mechanisms for better caretaking of citizen having rare diseases. It was formulated in 2017 and notified by the government in march 2021.

SIGNIFICANCE
1) The policy is pegged on principle of inclusion, which means, it is binding on a welfare state to secure the wellbeing of every citizen, particularly those unable to help themselves, irrespective of wheather they constitute a critical mass or not.
2) It offers financial support for the one-time treatment of upto Rs. 20 lakh and also introduces a crowdfunding mechanisms.
3) It provides early detection for the management of rare diseases.
4) Also creates a registry of rare diseases.

CONCERNS
1) The document classifies rare diseases into 3 groups and (Group 3) has diseases for which definitive treatment is available, but costs are prohibitive, have been categorized lower and hence would qualify for lower support from the government.
2) Rs. 20 lakh seems to be quite less and does not cover the actual costs of treatment, according to the caregivers of patients.

WAY FORWARD
-There is a need to support the development and commercialisation of drugs for treatment, and improve funding for reaserch.
-If the Centre can extend the cost-sharing agreements that it has worked out with Kerala, Tamil Nadu and Karanataka, with other states too, its share of annual costs will be halved.
-Requires sustainable treatment support for patients with Group 3 disorders.
