All symptoms and signs give clinical features of Rt. lower lobe pneumonia. The tracheal shift to the same site will indicate significant atelectasis.

I was thinking atelectesis due to the tracheal shift on the affected side, but am wrong. Fever, thick yellow sputum are sign of a bacterial pneumonia.

Pneumothora with onset of pneumonia right side .
Tachpnea, tachycardia and Hyperresonnant = pneumothorax
Increase fremitus and fever indication infection

PNE would cause fluid causing atelactasis on the affected side atelactasis causes the shift towards the same side

Loss of volume in the RLL due to consolidation secondary to pneumonia. The consolidation is evident by the bronchial breath sounds in the RLL and tracheal shift toward the affected lung.

Pneumonia. I didn’t know the answer until you said increase fremitus and a hypo resonant note with sputum. Curious on how much tachypnea and tachycardia played into this diagnosis? How do they play into diagnosing a patient? Awesome video though!

It sounds suspiciously like what I had when I was a toddler. Thankfully most cases like this don't require trachs anymore. Good to see you working hard Joe!

Unilateral lung disease as trach shifted. Sputum fever tachypnic indicates infection. Breath sound auscultation percussion indicate location of infected lung. R LOWER lobe lung infection

Right lower lobe collapse. Maybe due to sputum plug or endobronchial lesion.
