very clear video and learned a lot. I want to use it for our club for sending a form to all our members. they have to fill in their first name and last name and their Id number from the club, most members don't know their ID #.
there is however a list [xlsx] of all the member with this information. Is it possible when they type in the name and first name the form collects the info [ id # and email address] from that list?

How do you remove the bit in the footer where it says "The owner of this form has not provided a privacy statement as to how they will use your response data. Do not provide personal or sensitive information."
