This also happens a lot in engineering. Managers have literally asked me to make a brand new, disruptive version of a product... without changing anything at all from the original... for logistic purposes, of course. In many companies, the common logic is: when you want to improve something, do it in such a subtle level that time and money are minimal, and then they end up only approving changes that indeed are so subtle that, they obviously conclude, it's not even worth the time and energy to implement them, so the logic conclusion is always to do nothing at all. This goes on and on until some major reason forces them to finally make an upgrade and hell breaks lose...
