If you build a form in MS Word using tables it's so very easy and it allows for spell checker along the way. Pretty straight forward.

I have spent hundreds of hours learning how to use excel and word for work. Excel is by far the SUPERIOR app. Although I can now count myself as one of those who know how to prevent messing up everything in word in case you forgot to add a period.

For the phone number, instead of making a custom format you can go under the Special format and there is a built in one for phone numbers.

You can also choose for having a digital fillable PDF form. Its works great.

Cool tips! But personally I still prefer the alignment to the left for “name…etc”.
Any good ways to do that with an underline in excel?

Either MS Forms or a UserForm object in Excel would be better suited for this task.

(How are you going to get the data out of (Edited) Word to another application?)

Why not use something like google forms? Seems like it would be a simpler choice here.

Where do you save the data to? Do you type the next person over the first person without clearing it. Use Forms instead.

Also, in word instead of typing a line with underscores just use a line. Lines don’t move. LOL!

Umm.. Or you could use a table in Word and hide its borders? Or if you are a total beginner, you could simply underline text?
