I think something like this happened to me as well, even though it was in rebirth against Hush. I had Ludovico+Brimstone+Gnawed Leaf, so i decided to stay still for most of the fight as I still had little experience against that boss: to put it simply, Hush went underground, only to go underneath me while Leaf was active and instakill me as it re-emerged. So yeah, skill issue for not knowing that i should've avoided something that theorically shouldn't happen
