I’ve been suffering with this for about seven years now, they said it was sciatic pain however, I feel it’s more piriformis syndrome type of situation, I have no injuries or surgeries that would have caused or aggravated. This, I’ve gone for many epidural/pain management injections, and after several months, it comes back and it feels like my right leg is on fire I cannot sit, getting up from seated position can be quite a challenge. It feels like I have a tourniquet on my ankle all the time cutting off circulation. I’ve gone to orthopedics, physical therapists, acupuncture … And nothing seems to quell this pain. Sometimes it’s all I can think about all day long. I’ve seen some very good doctors, but no one seems to have an answer. I am not overweight I am not diabetic. I am physically fit for my age, but I just can’t imagine that this is how I will have to live for the rest of my life“ I just have to keep searching here in New York… And yes, I’ve even gone to chiropractors and had massage therapy also

Hi, piriformis originates at the sacrum an inserts at the greater trochanter. Can you please explain why the knee needs to be extended or why the knee position should matter at all?

I have pain in outer side of the thigh while doing such as clam shell exercise.
Without stretching, the pain sometimes can be also in the gluteal area.
What is possible name of the situation?
Thanks @Physiotutors

is there any benefit over a SLR-like piriformis testing, which from the joint angles is exactly the same?

Do i need to do all the test for deep gluteal syndrome like active piriformis test and this one and the others.. or only 1 is enough or how do i know if 1 test is enough..and regarding the positive signs should it be gluteal pain with paresthesia or is it considered positive even if just gluteal pain alone?or paresthesia alone?
