The tests above for piriformis syndrome can give a false negative. The reason is that flexing the hip (leg forward) causes the apex of greater trochater to rotate posteriorly. The apex of the greater trochanter is the location of the insertion of the piriformis muscle. Thus, flexing the hip relaxes the muscle which prevents the pain associated with tensioning it by internaly rotating the femur. A better test is to rotate the femur internally while the leg is in neutral position or slight extension. This slight extension rotates the apex of the greater trochang;er anteriorly, so puts slight tension on the piriformis muscle. This alone may cause pain, but if not internal ration of the fenur will.

A cause of piriformis syndrome not mentioned in most sources is weakness of the gluteus medius - GM. This weakness stresses the piriformis muscle - PM (resulting in its inflammation and sciatic pain) is because the piriformis is a posteriorward extension of the GM. If the GM cannot pulldown effectively on the pelvis to stabilize and balance the pelvis, the next muscle to do so is the PM. But the PM is not located at an effectice angle or location to do this well, resulting in its injury, inflammation and sciatic pain. The point here is, if all normal measures fail to improve the symptoms, consider exercises to strengthen the gluteus medius.

I like the conciseness of your videos. I am studying Sports Massage for the higher level and apparently we would need to do these tests for our assessment. Your videos are short, precise and not too much technical jargon. Thank you.

I had this the whole time I just didn't know the name of it. This is so painful to me when I have it and when the pain comes I get paralyzed where I can't move for a few hrs and it's hard for me to breath. Do anyone have any tips for me because I been up since 3am? When I put ice and heating pad it trigger it even more and it gets worse. Thank you.

Just a quick thought, could you not get the patient in a supine position with the knees flexed to 90 degrees and patient performs internal rotation of the hip to look at piriformis? I suppose then you can compare both sides depending on the position of the legs?

If this test comes out positive can I opt out a herniated disc as the cause of the pain? In my case, a CT scan showed clearly a herniated disc a few years ago, the symptoms were stronger back than..
