Once in a university maths exam the exam was stopped because there was an error in one of the questions. Those students trying to answer it wasted a lot of time. Extra time was given. I have wondered if the interruption also caused other errors.

These types of things happen more often these days. There is less editing and oversight. I was monitoring a district wide math test and noticed several errors because the students kept asking me about the questions. I asked the math coach and she said to have them do their best guess. I said, "but none of the answers are correct." She said she would just use the answer closest to it. So essentially children who guessed would be marked correct but those who knew all of the answers were wrong and left it blank would be marked incorrect. I think they had taken off the answer: E: None of the Above but forgot to cut those questions.

This was likely to generate revenue, failing them so they have to pay again.

I complained about the final after I failed an accounting class. The professor just gave me the answer sheet in response for the retake because no one could figure out how to correct the errors of whoever had input the answers

That sounds eerily familiar to my kids NJ elementary school. The teachers don’t actually correct the answers themselves they use a provided sheet with correct answers on it. I experienced this first hand when my son in 5th grade knew his answers were correct. The teacher later looked into his answers and said he was correct and the answer sheet was incorrect.
