How it would actually happen:
The moment the missiles are launched, over 100 launch warning satellites would all simultaneously send a warning signal to multiple compand and control facilities including NORAD, the Whitehouse (or wherever else the president might be), The president then sends our launch facilities a lauch order involving a launch code. The president is then moved to shelter.
(At this point, it has been between 2 to 3 minutes since the Russian launch.)
Our own nuclear forces now check their confirmation codes to see if they match the launch codes, this is to make sure the launch order is authentic. (This takes about a minute) after confirmation, the launch keys are turned, and about 45 seconds later, (now somewhere between 4 to 5 minutes since Russian launch) 3000 of our own nuclear missiles are airborn and headed towards Russia.
10 minutes later, the first Russian weapons would reach their targets. (4 to 5 if the weapons are hypersonic).
Another 5 minutes, and the first American weapons would reach their targets.
About 97% of all weapons launched will reach their targets, while roughly 3% are intercepted.
At this point, both America and Russia would both be crippled.
While that exchange is happening, the American navy would be hunting down and destroying Russian submarines, and our own submarines would be moving into attack positions.

After the first exchange has finished, the president is then taken to the mobile command and control center, which is a heavily modified jet containing everything needed to communicate with what remains of the armed forces. Once airborne, the president then gives a second round of launch orders to our submarines, which then complete the confirmation of the order, and launch their their missiles at any remaining Russian targets. (The remaining Russian submarines would be doing the exact same thing).
After the second exchange, America and Russia would both be completely destroyed, the land would be heavily iradiated.
Another 48 hours later, secondary fallout would begin to fall back to the surface, spreading radiation around the globe.

Basically nobody wins.

This would be accurate if we didn't have submarines and allies with nukes and early warning detection systems and missile defense systems and command and control centers designed to withstand nukes and assume Russia has actually been doing the maintenance on the missiles... But yeah other than all that very accurate.
