I always liked to imagine Death in the DC Universe uses so many different forms and appearances since mortals and superpowered gods in the DC Universe only have so many comprehensions of what Death is; either Death is a beautiful young woman guiding you to the other side as shown through Death of the Endless in the Sandman comics, or maybe Death to a speedster looks like The Black Death, or maybe to a New God The Black Racer is the form of Death they see or for different Lanterns and aliens they see Nekron as part of Black Lantern Corps

To sum it up- black flash is death for speedsters, black racer is death for the gods. However both are aspects of the true Death - Death of the endless (the endless are abstract entities meant to represent immutable forces that govern creation and life).
When barry became the "black flash" during the new 52 he was really the host for the spirit of the black racer, hence why he became the GOD of death
