D-dimer (or D dimer) is a fibrin degradation product (or FDP), a small protein fragment present in the blood after a blood clot is broken down by the body’s own clot dissolving mechanisms called fibrinolysis. It is so named because it contains of two cross-linked D fragments of the fibrin protein.
Under normal circumstances, D-dimer is not present in the blood or if it is, it is at very low levels. D-dimer concentration may be determined by a blood test and it is raised in thrombosis or clot formation. Since its introduction in the 1990s, it has become an important test performed in patients with suspected thrombotic disorders such as Deep Vein Thrombosis and Pulmonary Embolism. While a negative result practically rules out thrombosis, a positive result can indicate thrombosis but does not rule out other potential causes. So the D-dimer blood test used to exclude thromboembolic disease where the probability is low.
Doctors may request a D-dimer when a patient has symptoms of DVT, such as leg pain, tenderness, swelling and discolouration or when a patient has symptoms of Pulmonary Embolism, such as breathlessness, cough, and sharp chest pains. D-dimer is especially useful when the doctor thinks that something other than DVT or PE is causing the symptoms. It is a quick, non-invasive way you’re your doctor to help exclude abnormal or excess clotting.
When a DVT is suspected, the D-dimer blood test is used as part of the Wells Scoring system. In people thought to be at low risk, a normal D-dimer blood test excludes the diagnosis. Just a note of caution, the D-dimer test may be slightly raised without clotting problems in people with rheumatoid arthritis, who are pregnant or who are being treated with oestrogens.

Thank you for your expert advice. My D-Dimer was 556, acritical level 2wks ago. Now it is in the 300s range. However, I do have systemic osteoarthritis with bone spurs in my upper/ lower extremities. After research, I wonder if I have rhabdomyolysis (the rapid destruction of skeletal muscle in my bloodstream.). I will follow up with a physician soon. It is quite frustrating that some primary care docs are dismissing the critical levels as normal due to arthritis. However, my opinion is there is nothing normal about critical levels.

Is it still a useful test for women on birth control? Or do you skip right to ultrasound since estrogen raises d dimer?
