Plasminogen is converted to plasmin, thanks to tPA/urokinase...Plasmin will degrade the fibrinogen into fibrinogen degradation products, the fibrin into fibrin degradation products, and the stabilized fibrin into d-dimer.
D-dimer is a subset of fibrin degradation products (FDPs).
Plasma level of D-dimer is elevated in cases of DIC, DVT, coronary thrombus,...etc.
The presence of high plasma levels of d-dimer proves that there were some stabilized fibrin fibers in the plasma.

im thinking about the hemostatic balance between procoagulant vs And I just wanna know your thoughts on elevated D dimer in DIC. increased D dimer means fibrinolysis, but DIC is making clots/thrombosis everywhere, so why do you still see elevated D dimer in DIC patient? I get that DIC patients have decreased level of fibrinogen, because they are being used to make clot and they also have thrombocytopenia because platelets are being used to make clot. But I don't get why D dimer level is high? It should be low, right?

What bothers me is that I always read that D-Dimer is a good negative and not as good a positive test...thus a negative D-Dimer always excludes the presence of a clot which is contrary to what you have mentioned in the example !!! what do think?

Hi Doc, my father in law is in hospital with COVID he is 70yrs old and also has diabetes, he on oxygen as struggling to breath he had D Dimer test today which was 10000ng/ml, plz advise for treatment as we in uk and he is in India and injections for thinning his blood is not working..
