This is actually tragic. Brings to mind a case where a patient was admitted for a stroke and ended up with stroke plus severe traumatic head injury from falling in hospital bathroom due to inadequate manual handling while being showered. His attending was almost in tears when she reviewed him. Also brings to mind a surgical patient whose surgery seemed fine and was being discharged on day X as per the standard surgical pathway - despite the small hiccup that he lived alone, was previously independently mobile, but now couldn’t manage to walk safely from the ward to the lobby - not sure why it took a PT to point out that he shouldn’t just be sent home or to suggest someone look into why he could no longer walk normally. Just because a doctor is not from neurology or aged care shouldn’t mean that they get a leave pass from noticing basic issues like if a person can walk short distances without losing balance.

This reminds me of when they tried to send my grandma home with no rehab/plan to prescribe PT. She couldn’t walk on her own and previously was totally independent. All of the adults in my family had already used up all their time off work rotating to be with her in hospital. That was a nightmare

As an acute care SLP I literally laughed out loud at the ending because of how spot on you are. Sending this to all my coworkers as this is something we deal with daily with the baby docs.

I had someone like this . This pt had every team possible on their case. Then I managed to get everyone to sign off on them (8-9 specialist), RT, PT, and acceptance to the acute care facility, as well as the TB /COVID testing in a expected time frame prior to DC. Oh, yes even the ambulance ride setup to go through with the help of SW . Then you know what happens no more than 30 minutes before DC. She jumps out of her chair and turned the chair alarm off . Oh, even took her oxygen off and hit her head . 🥺😕😳😩🤨🤨 so guess who stayed over the weekend in addition to the 5 months she had already been there. 🤣
Update she was fine . A sweetie that I adored but every one was rooting for her to get home to her animals and family . She was not harmed and managed to push through.

I can imagine a similar situation with a discharge planner. The hospital i used to work at has a designated staff for discharge planning and they are tasked to ensure that anything a patient needs is sorted out before discharge. They also usually advocate for clients that are not safe to go home yet. That's the one reason I didn't go for that position as they get pressure from doctors and hospital to discharge patients then pressure from home care to keep them in hospital as their unscheduled care needs are too much for homecare supports.
