As someone pointed out down below, he’s not modulating or changing keys very often here, it’s just secondary dominants in the key of D major. There is a brief change to b minor in the middle but the rest can be viewed as tonicizing the ii or the V chord generally speaking, briefly the IV chord near the end. I would simplify those e: or A: modulations to iihalfdim/ii-V7/ii-ii in D or ii/V-V/V-V staying in D as they occur.

I had a dream with this piece this morning, and the first I did when I wake up was to search it in youtube. How happy I felt to see this one. so slowly and full of that spiritual character that I think Bach thougt. Thanks a lot, and I'll try to play the first two or three bars of this musical gem, at least.

I'd say the first one in bar 12 is E(half-dim)7, instead of G#. The tritone has a good resolution, then! :)

Strictly speaking, this is more harmonic labelling than analysis, per se. Nothing wrong with that, of course (and a very thorough job has been done here), but it's only a preliminary to real harmonic analysis.

I looked at the first 3 bars. The first chord in bar 3 looks to me a VII in e, not a II. Because the e in the first beat resolves to the d# in the second beat, so it is a suspension. Which makes it very rich. In the score these 2 notes are played by the violas.

Bar 17; that's a bit finer than Em7/B, it actually goes Gadd9/B before hitting that passing Em7/B. Still, on the strong beat it's a Gadd9/B that resolves on a passing Em7/B. I saw a few other similar mistakes here and there.
