If yall wondering why he does it the start of the comic kinda explains: the narrator says "no cruelty is too small for the terrible king either. And may even offer GREATER pleasure." To put it simply, he just thinks its funny to mess with people and show them he can do whatever the hell he wants to them and they cant do anything about it. David constantly worrying about Thanos stepping in to make his life miserable, every single year, on what is supposed to be HIS day, amuses Thanos. Thanos even says when he returns after kissing David's 30th birthday, "I love these meetings. There is nothing more important to me, do you understand that?" Destroying a single nobody's life is the most amusing thing to him because it truly shows how seperate he is from the average person, and shows how miserable he can make people feel.
