1. Depends entirely on which canon you’re following. The Kaleesh have plantigrade feet in their original design for Grievous’ transformation comic and later for the Old Republic video game. The only time we see digitigrade Kaleesh was a later retcon to make them seem more like his cyborg appearance, but the design was originally meant to simply enhance the General past his biological limitations.

There’s also another factor for his cough and that’s because sure the battle of Corusant, Grievous went to collect Palpatine and bring him back to his ship, and during the evac, Mace Windu used the force to crush his chest.
