1/(x-4) + 4/(x+2) is not equal to (x^2+3x-4)/(x^2-2x-8) @4:55. You need to use long division first to get the degree of the numerator to be strictly less than the degree of the denominator: (x^2+3x-4)/(x^2-2x-8) = 1 + (-x+4)/(x^2-2x-8). Then you can use partial fractions on the second part.
