Someone else said that kaigaku represents rabies, but there are good arguments for both. Main rabies arguments are the clawish marks on his face, his teeth that are much more pronounced, and that he used to drink dirty water as a human, which is a way animals contract rabies.

You could argue that Rui represents chronic pain. He completely falls over on his bad days. But on a good day like when he was trying to get Nezuko to join his family he can do so much more. A little bit of speculation though.

I think as mental disorder, Enmu inspired by Schizophrenia because he has some symptoms of Schizophrenia:

People in the first flare-up of Schizophrenia cannot distinguish between dream and reality, the same can said for Enmu's because according to Mugen Train Taisho Secret, Enmu had a tendecy to not to be distinguish between dream and reality since childhood.

People in the second flare-up of schizophrenia harm themselves or people around them, the same can said for Enmu's because according to Mugen Train Taisho Secret, as an adult, Enmu misused hypnotic therapies to convince terminally ill people that they had recovered their health.

Schizophrenic people have vacant facial expression, the same can be said for Enmu because he has vacant facial expression and it is only clear from his mouth what emotion he is feeling at that moment and he has very dull voice.

Schizophrenic people have socially inappropriate behaviors, it can said for Enmu because he misused hypnotic therapies on terminally ill people.

Ouch٫ modeled after an STI. Honestly٫ I've always had a soft spot for Daki and Gyutaro regarding the sibling thing. My twin died٫ and I'd have given anything to protect her. Syphilis does make sense. Either that٫ or a bone disorder.

I reckon Kyogai is cancer. Yes, I know cancer is big and Kyogai isn’t even a Lower Moon, but get this: he was underestimated and overlooked by the person who dismissed his writing, and then Kyogai killed him. Just like how symptoms of cancer can be dismissed as another illness, and by then it can be too late!

On the mental sickness side, Hantengu could be multiple personality disorder, while Kyōgai could be anxiety or imposter syndrome

Huh, it makes sense. Douma is a disease because nobody likes him and since he has no emotions. Kokushibo is probably the disease to make you try harder at your goals.

also with doma and TB as his disease it was also known as a beauty disease cause it made people pale and slim which people back then found beautiful. And he seems to care about his looks/is a very pretty person. Also another name for TB was /consumption/ back in the day, hinting to when he's feasting and has blood all over his mouth.
