brain rewires itself after severe brain injury

Hello there, I am Dr. Carlo Oller. I am a board certified
Emergency Physician with 18 years of clinical experience.
I post medical education videos on my youtube channel DRERTV.
I have a special interest in brain health and maximizing brain function.
You see, my wife had herpes simplex encephalitis 6 years ago,
which resulted in her developing and Acquired Brain Injury (ABI for short)
and since she has had problems with brain function, specifically brain recall
and memory. Since then I have read almost every article that comes across
related to brain function and how to improve it. Lately I’ve seen an upclick
in the number of articles related to brain function, mainly because patients
who are recovering from COVID-19 are experiencing similar symptoms to that
of my wife. They have coined the term Brain Fog to describe the self awareness
that their brains are not functioning at optimal capacity, that it feels much like it
is in a ‘fog’ state. They too have sustained a form of ABI to different degrees
and there is increased demand to find out more about the condition and how to
help it. My goal is to create medically sound education for people searching for
answers about ABI (whatever it was caused by), encephalitis, and COVID-19 related
Brain Fog. So go to www.DrER.tv and subscribe to the newsletter so that you can find out early when I finally create a channel / webpage specifically for brain function education in the context of ABI.
Now lets talk about this recent article that was published recently on the Lancet of Neurology in March 2021. The title of the article is, “Brain rewires itself after injury ‘ on the edge of what’s compatible with life’”. It tells the story of a young teenage boy who, because of some motor function quirks when catching a baseball, the family began to suspect that something wasn’t quite right. She went to few doctors and medical providers before a neurologist finally ordered a CT scan of the brain. The results were astonishing!
The picture in the thumbnail of the video, and I will show you right now, shows the brain of the 13 year old on the left and a normal brain on the right. Brain looks white/gray on the scan, fluid looks black. As you can see in the picture almost 70% of the brain tissue has been replaced by fluid on this scan. The neurologists concluded that the patient had a significant stroke around the time of birth. “It was the largest stroke I’d ever seen in a child who hadn’t died or suffered extreme physical and mental disability. Furthermore, the extent of the injuries may be on the edge of what’s compatible with life.”, said the associate professor at the medical center he was being evaluated.
Review of the patient’s medical records show that he was admitted to the NICU shortly after birth with fever and respiratory symptoms. He was very ill for about 3 weeks, but a neurological event was never suspected, and patient was discharged home after he improved.
That sounds to me like this patient had viral encephalitis of some kind causing the ‘upper respiratory infection’ and fevers. But because the child was so little the normal complaints of severe headaches and altered mentation went unnoticed by the medical providers.
And unnoticed during this child’s childhood, but he probably struggled to achieve average function in school. Now remember this kid was in 7th grade before this diagnosis was made. In fact, it was probably due to the fact that this child was treated as a normal child that forced the brain into functioning at a much higher level than what it should have functioned as.
This patient is now an adult and earned an associate’s degree from a technical college and works full time rebuilding diesel pumps for trucks.
This case illustrates what used to be science fiction in medical school. You see, I was taught that the brain has a limited # of neurons. Once the tissue in the brain forms, you can’t get any new cells...in other words, the brain cannot rebuild itself. But now we know that with adequate diet, sleep, and exercise the body secretes BDNF (brain derived neurotropic factor) which helps make the hippocampus (an area of the brain in charge of memories) enlarge and grow in cell number and size. It has also been observed that the pre-frontal cortex can grow in response to a healthy lifestyle.
So if you or a loved one has suffered any kind of ABI (stroke, encephalitis, concussions, or other medical illness that results in less than optimal function) remember that there is hope. Keep pushing yourself to as close a normal function as you can. Eat / sleep / and exercise and the results will come over time.
Stay tuned to future videos like this one related to brain injuries and optimal brain function.
