We are some hospice patients claims as our provider saw them. My query is patient has advantage plans as primary and Medicare also. so can you pls confirm we need to submit claims to the advantage insurace or the medicare?

I m in hospital billing wherein commercial insurance or hmo denied the claim for hospice ...so we have to find the hospice correct mailing address then along medicare we bill the claim...ye steps toh apne bataye hi nhi

Sir, what if primary payer lets say UHC and they dnd for same reason COB09, Then shall we bill to UHC OR medicare part B, with GW Or GV modifier..?
