#MODIFIER: The 2 digit code, that provides an additional information to the service/treatment/ procedure (CPT code) without changing the original meaning of the service.
Mod Format: 2 digit Numeric or 2 digit Alpha or 2 digit Alpha Numeric.
The consistent relationship of CPT-MOD helps to Providers by getting payments from Insurance companies.
Example : 93010-26, 93010-TC, 93010-LT, 93010-RT, 93010-50, 99213-24,99213-25……..etc.

* The inconsistence relationship of CPT-MOD is the reason to deny the claim stating CO4: Cpt code inconsistent with Modifier.
Pre-call analysis: -Proper analysis required, verify previous Dos.

Action s: 1. Task to CODING ASSISTANCE/Review (with AR caller notes).
2. RESUBMIT the corrected claim with valid CPT-MOD relationship.
