Dermatitis herpetiformis, also known as Duhring’s disease, is a chronic skin condition caused by a reaction to gluten ingestion. It causes clusters of small blisters, bump and also itchy skin conditions. Symptoms typically start at age 30 to 40 but can happen at any age. This lifelong condition is rare in children. It affects more men than women.

Causes of dermatitis herpetiformis

Despite its name, the herpes virus doesn't cause dermatitis herpetiformis. If you have celiac disease and eat gluten, your intestines make an antibody called IgA in response. This chemical flows into your bloodstream and builds up in blood vessels under your skin. It triggers the DH rash.

Symptoms of dermatitis herpetiformis

Dermatitis herpetiformis looks like a cluster of itchy bumps that can be easily confused with acne or eczema. Blisters can also form, and you could be misdiagnosed with herpes.

Dermatitis herpetiformis commonly occurs on the body area include knees, elbows, buttocks and hairline.

Dermatitis herpetiformis is not contagious.

Can dermatitis herpetiformis be misdiagnosed?

Sometimes dermatitis herpetiformis is mistaken for herpes, scabies, papular urticaria and other types of dermatitis. Your healthcare professional may need to test your skin to determine an accurate diagnosis.

Treatment options for dermatitis herpetiformis

Treatment will depend on your symptoms, age, and general health. It will also depend on how severe the condition is.

Usually, dermatitis herpetiformis is treated with a gluten-free diet and an oral antibiotic called dapsone. If dapsone doesn’t help, your healthcare provider may prescribe sulfapyridine or sulfasalazine.

Patients are usually managed by a dermatologist for the management of dermatitis herpetiformis and a nutritionist for dietary counseling.

I had the blistering rash - on my face (the ooziness lasted for days on end) and the blotchy rash on my neck and my outer upper arms. After four dermatologists and countless pills & creams, it took a GI to diagnose my celiac. My derm at the time said she didn't think it was celiac because my "rash" wasn't where it normally presents if it's celiac disease. Thanks for sharing this information!

Thanks for the content. Even it is very rare, people who are diagnosed with Morbus Duhring are really happy about any information about it. My deasese startet in March 2019 out of nowhere and it took the doctors almost one year for the diagnose and and then I needed another year to be symptoms free. I descided to not take dapsone or any other meds. Strict gluten free diet is obligatory. Fasting, green smoothies and everything anti-inflammatory helps a lot to get rid of the symptoms more fast.

This video doesn’t emphasize how the few of us who have this are misdiagnosed most of the time. I’ve seen so many doctors for help for 30 years. “Allergies” is the usual misdiagnosis. “Psychosomatic” is the other. The later is the humiliating one. It’s been 30 years and I don’t have an official diagnosis but I know this is it. I’m sitting here suffering from this now. I try to be careful not to eat wheat, but sometimes it comes in sly forms “cellulose gum” in corn tortillas hit me this time. Also, for the first time, the lymph glands in my inner elbow swelled with blisters inbetween. Just know from one who has it, don’t waste your money for a diagnosis because most medical professionals don’t put out effort on those that don’t fit in to their easy boxes. This is rare and they won’t recognize it. Just stay away from wheat the best you can, cry when it tortures you, rest when you can, and face reality that there is no cure so manage well with absolutely no wheat. Don’t eat others food and do your research on ingredients that could be made from wheat such as cellulose gum.

in differential diagnosis as mentioned i think scabies, herepes or insect bite or neurodermatitis are all similar but what can be done to the patient is Biopsy (of vesicle before it is scratched) showing – Subepidermal blister with neutrophil infiltration in dermal papillae

Gluten is a lectin. Keep in mind there are other foods that contain lectins too. So, other foods containing lectins may cause DH as well. A reminder that certain soaps, shampoos, make up, etc. contain gluten. I have DH and Celiac disease.

I have Graves disease & I'm pretty sure I have this condition, too. I've got the same sores as the first picture (top left), and I'm constantly getting rashes and swollen legs. I'm not even 40 yet, and I only weigh 86kg.
