How to Make Root Cause Analysis in Excel (Cell-based Fishbone or Ishikawa)

This video shows you how to create a cell-based Root Cause Analysis template in Excel so you can brainstorm the cause of issues (or opportunities) with your team. Also called a Fishbone diagram or an Ishikawa diagram, after Kaoru Ishikawa at the University of Tokyo.

Typically causes are put into four buckets as "PIPS" - People, Information, Process, or System causes. You can also use "MMMM" which is similar - Man (or Woman), Materials, Method, or Machine.
