So what happens when the nanobot breaks the fibrous cap, which is the only thing that separates the liquid plaque inside the atheroma from hitting the bloodstream?... that is not what an atheroma looks like, it is not just a pile of junk on top of the this is the most typical way a heart attack starts, the fibrous cap I think it will take a more nuanced approach to remove plaque, probably some sort of bio-chemical or genetic approach
