Hypotension is one of the worst vital signs we can see as the patient can be at risk of not enough blood/oxygen reaching all the bodies tissues. Shock, illicit drug use, cardiac emergencies and dehydration can be some of the culprits that can cause hypotension in your EMS patient.

Cardiogenic is often a cause and should be evaluated quickly due to its high lethality. Under cardiogenic there are three subtypes: rhythm disturbance, weak pump and valve apparatus failure.

Cardiac meds too- Ask what meds they took and when. The time of onset may be a factor

POTS is also a possible reason for hypotension. if the arteries can’t constrict properly then blood pressure can drop.

This might be a stupid question but will a patient have a low pulse rate too if they have low blood pressure or can they have a rapid heart rate and still have low blood pressure?

But please don’t assume that EVERY case you come across is a drug od. I was in a seizure and had low blood pressure. I have an extensive history including MIx4 PE epilepsy with staticus epilepticus paraplegia sick sinus syndrome pots dysautonomia gastroparesis and other smaller issues. I was unconscious and AMS the first thing that ems did was give me not one but two doses of narcan with no effect. When I was awake but not alert I began to tell them I didn’t want any treatment and instead of backing off they tried to set me up for a central line because I have little to no iv access and I had already blown multiple iv try’s and was covered in swollen areas and bruises on my arms. They used soft restraints to try and force care on me. I’m a DNR/DNI and have advanced directives on file with the ambulance company both local hospitals and on my wheelchair yet all of that was overridden. I’m now looking for someone to represent me in a lawsuit against all of them

Tell them to take a lemon, put a whack load of salt on it, and eat the lemon. Them drink 32 oz of fluids. Always works for me.
