I will often blow ALL of that amalgam out of there first, and then do a nice bonded core buildup before having the endo started. If I’m referring it to an endodontist, I know they appreciate that so they don’t have to deal with amalgam particles in canal spaces, and have an easier time getting nice rubber dam isolation. Then when the endo is done, the crown prep is also easier because you’re confident the buildup is a nice foundation for the crown. Those are some HUGE and failing amalgams!

Good video, great tips. I am in the "remove all the amalgam" camp for better visibility, light entry, acess restorability and better seal. Love you flare the mesial's coronal, do that a lot to get straight line access.
Another pearl to add is to drop a drill in and prep out a post space in the distal BEFORE you obturate. Makes it easy to do a post later as you have only to remove some GP. 27th yr of dentistry for me and always learning something new.

Remove amalgam, find MB, rebuild coronal with resin for better seal and to prevent amalgam breaking off during healing phase, C/S, CaOH to length, sealer tape/teflon and close with RMGI. My preferred method. Thanks for the vid. Nice presentation.

Krasner and rankow should be the first lecture of an endodontic syllabus! I lecture to a lot of newly qualified dentists and few have heard about it or recognise it’s significance.

OH and I have also had amalgam go straight to the apex. Reminds me of panning for gold concept.

I have really bad experience with doing rc under Amalgam. In 99, 9% of the time the margins aren't leakproof thus ruining the desinfection process and when opening old AmFs you usually find endo "mud" underneath preventing you from just doing a crown on this tooth.

🤯WHY ON GOD'S GREEN EARTH WERE THOSE TEETH FILLED WITH AMALGAM!?!?! ☝🏻There's an amalgam overhang mesially of that obsurd filling...that amalgam filled tooth contracts and expands with the ever changing temperatures of the mouth AND bite strengths 🙄🤷🏻‍♀️
Those teeth should have had a buildup and covered with a crown
