How to create a Quick Poll using Microsoft Forms in Outlook

Can I create the same in Outlook PC App ? How to add Forms add-in inside my application ? and could I create a complete form inside my email message / already created form in my MS forms ?

Is there a way to make the answers private so no one can see the results other then the employee whom made the poll?
