Dr. Chip Grant founded Watershed Health after building a career as an interventional cardiologist with a passion for improving patient outcomes. Prior to his role as Chief Executive Officer at Watershed, Dr. Grant focused his practice and research on preventative medicine, chronic disease management, population health, and digital medicine. The Watershed Heath's purpose has been simply to improve patient outcomes and its platform is best known for reducing hospital readmissions. More than 1000 health plans, health systems, and post-acute providers trust the Watershed post-acute care platform every day.

Kudos to Dr. Grant for tackling a huge change effort focused on positive outcomes and for refusing to settle for hitting the “easy” button! I have long been a fan of Dr. Grant’s medical acumen and desire for excellence when it comes to patient outcomes. In 2013 he was in charge of my husband’s care in the CICU at Springhill Hospital. I was struck by his being hyper-focused not just on patient care but on teamwork. Even though the outcome was not what we all hoped, we walked away secure in the knowledge that under Dr. Grant’s passion for excellence treatment style always carried the day. On a parallel track, as a mental health provider in an outpatient integrated care center where doctors and behaviorists worked together to try to increase positive outcomes for patients, I always knew something was inherently wrong with our model to evaluate patient engagement. We used a “readiness to change model”that scored the patient but at times ignored the provider’s readiness to be actually engaged. I sometimes felt less than satisfactory outcomes were placed solely on the patient without any provider engagement analyses. This is a very exciting platform. Makes me want to come out of retirement for the third time!
