Investigators are convinced that contaminated titanium in United 232’s fan disk caused its July 19, 1989 crash. This led the NTSB to push for a more stringent manufacturing process, to ensure it never happens again.

Of the 296 people on board the ill-fated Flight 232 when it crashed 31 years ago this month, 112 passengers died and 184 survived. Only 9 years old at the time, Brownstein was one of the survivors.

Nitrogen and Oxygen in the Titanium alloy this increases the chances of cracking and when the crack got really big the fan disk can no longer hold the stress and then it breaks. The shrapnels of the disk severed all the hydraulic lines which is the nervous system of the airplane. The pilots manage to get the airplane in position to land but just as they aim for the runway the right wing drops and as a result the aircraft smashes into the runways demolishing the right engine and breaking into three pieces: Cockpit, Tail and Main Fuselage.
