Keep in mind, he possibly looked a lot worse than what we see in the painting, because for obvious reasons painters had to make him look good

If i recall correctly, these paintings were the "photo shopped" versions. As in made to make Charles here look way better.

So I was reading about incest, and apparently, 1st cousins have 2-3% higher rate of birth defects in their children. 2nd cousin couplings result in a negligible increase in birth defects, and once you get to 3rd cousins, they share so little DNA (0.78%) that there is no increased risk of birth defects. Shockingly, even when speaking of children who are the product of sibling incest (roughly 50% shared DNA with full siblings), the children usually don't have any manifested defects. Apparently, the biggest problem with incest is when it's successive throughout several generations, as seen here with the Habsburgs.

Edit; most people are surprised to learn that incest is actually very commonplace outside of the West (cousin marriage, not direct relative incest) in the modern day, and even in the West, cousin marriage was quite common until the very end of the 19th century.

I remember seeing somewhere that Charles II had a Inbreeding Coefficient so high that his parents could have been full siblings and they would have lowered the Coefficient.
