A non healing ulcer is a chronic ulcer which is not healing with repeated dressings, with antibiotics, and with debridements. Most of the time non- healing ulcers in our country are because of diabetics and diabetic foot. It can also happen because of blocks in their arteries because of peripheral vascular disease. These two are the main reasons. So if the patient has a non-healing ulcer that patient has to be investigated. We need to treat them for diabetic or diabetic foot. We need to do an arterial dopler scan or a CT angiogram to see whether they have got any blocks in their arteries. If there are they can be managed with an angioplasty or by a bypass there by allowing blood to flow into the legs and the ulcers will start healing faster. A diabetic foot ulcer is a chronic problem. When the patient presents to us with a diabetic foot we give them counselling, we refer them to an endocrinologist so that they are properly treated for the diabetes. If there are other co-morbid factors or lifestyle factors like smoking, alcohol use. If there is any pus in the wound we take pus for culture. If needed we do an X ray or MRI of the foot added to that we do an Arterial Dopler scan of the led to see whether any blocks are there in their blood vessels. If the patient has got any extensive cellulitis or gangrene we will operate and remove all the pus area and the necrotic area of the foot and then allow the foot to heal. We also have a foot care centre where we refer them for off loading footwear which is very important for healing of diabetic foot.
