I'm American and I've been wondering #3 myself. It used to be considered extremely rude to be loud in the US, especially at restaurants. That started changing in mid 90s. Prior to that, it wasn't uncommon to get glaring stares, if your conversations could be heard in restaurants. If you didn't get the hint, it wasn't uncommon to hear someone tell them "no one cares about your personal life" or even more to the point "shut up". People/families would have conversations at their table, but it wasn't much louder than a whisper. It was what we used to call using your indoor voice. Now you almost have to yell, because the people sitting across the table can't hear or understand you over the volume of people sitting at a table behind them.

The whole traveling thing brings up an interesting conversation. Travelling to a different US state is like travelling to another country, mostly because most of them have their own unique culture and have things to see. And also US is like 10x bigger than most countries. A singular state is a size of a country in Europe or Asia.
