So what happened to Taki and Mitsuha after they reunite at the ending of Your Name? It turns out that they made a cameo in Makoto Shinkai’s next film, Weathering with you. However, the events that involved Taki and Mitsuha during that movie occurred one year prior to their reunion at the end of Your Name. In the Weathering with You novel, the protagonist visits Taki’s grandmother two years after Taki and Mitsuha reunite, and the protagonist sees their wedding photo indicating that they got married within 2 years of reuniting. The marriage was further confirmed by the author during an interview at the French premiere of Weathering with You. For whether or not they actually regained their memories? It has never been confirmed and it is left to our imagination.

The thing that was really bugging me at the end of 'Your Name', was that I didn't know what happened to them. I didn't even notice the cameo in 'Weathering With You' until I read about it in an article online.

The novel of ‘Your Name’ written by Makoto Shinkai, confirms that they regained their memories. In the final chapter titled: Struggling Magnificently, the two as adults are going through their lives with the words, ‘just a little longer’ and ‘just a little more’ being constantly stuck in their heads without knowing why. That is until they see each other on the trains, then they both think, “I remember!” “I wanted a little more time with him” and “I wanted a little longer with her” remembering their time on the mountain together. Makoto Shinkai has also confirmed that at the end of the story the two after hearing each other’s names remembered everything and went on to get married.
