I think Rules are changed now for fielder's Not out when it's been caught after deflecting from helmet or even helmet strip

What does touching the ground mean? Is it properly defined?

What if fingers are underneath the ball but the grass is touching the ball between the webbing of the fingers? Is the grass considered as part of the ground?

I thought you can't be caught if it comes of the helmet of a fielder or is stuck in it.
Also, the catch is not fair if person getting up grounds it while still holding it.
