All would do well to remember the tale of Timbles the small, who led one of the largest Waaaghs in Imperial history, consisting of well over a million Orks.
His glorious charge lasted for all of five minutes, before he tragically tripped on a rock, breaking his neck in the fall.
Young Gretchin still recount the tale, in the hopes that future generations will be inspired by his example.

Well since in fantasy we have Grom(goblin that started a WAAAGH) and the universes are bound and have common links then i think its possible to happen

I don't see Orks standing for a Grot ordering them around, even a Grot in a Killer Kan. They would just destroy the Kan, so the Grot wouldn't be able to control anything beyond a small tribe that had few enough Orks for it to be able to win the fight.

Now. If a Grot was very very smart, in a tactical/strategic genius kind of way, and it was part of a Waagh large enough that the Boss was also exceptionally smart, but not quiiite as smart as the Grot. That could work.
It would need a Boss that was smart enough to recognise that the Grot was smarter still, and to realise that the advantages of letting the Grot take command outweighed the inherent irritation of being told what to do by a runt. AND the self control not to squash the bossy little git. Maybe then the two of them could lead the Waagh, the Grot coming up with the plans, and the Ork giving the commands, while pretending the Grot is just a minion.

Thing is, if the difference in intellect was too much, it wouldn't be long before the Ork ended up killing the Grot because of the inferiority complex it would inevitably cause. For it to work the two would have to be almost as smart as each other, in which case it would be more of a partnership than a Grot leader.

I suppose they could lead a waaagh. But to do so is to convince the orks you are a bigger ork than them. At which point because the orks believe it, it becomes so and you simply become another ork warboss.
