Thanks! Great info! I tested my TYG and my index is 4.38 just under the cutoff of 4.49! It took me a little over one year to reverse my bad habits of eating and drinking, I lost 45 Lbs. and lowered my BP.
I used a low carb diet, One meal a day, Walked every day, lifted some weights 3 times a week, and cut out Sugar and Alcohol to minimum. I am 69 and hope to live a long and healthy Life

Super useful! Using the TYG index, it's the first time I got some idea about my insulin resistance. Using my blood test data over the past 10 years, I can see I went from the danger zone to safe numbers after changing my diet to whole foods and low sat fat with intermittent fasting.
However, there is one issue with the TYG index if anybody wants to follow the literature. The online calculators use the formula TYG = ln(glucose*triglycerides)/2 but most (not all) of the research papers use a different formula: TYG = ln(glucose*triglycerides/2). It looks like one source created a typo in the formula, which then propagated. Using the first formula, the normal TYG is a bit above 4, while using the other the normal is above 8.

Great content, Dr. Carvalho! Thank you. It's amazing how few physicians know of HOMA-IR, etc. or even discuss or factor in waist circumference. Also, for women and waist circumference, isn't over 35 inches considered high risk? And, for men, it's over 40 inches? Thanks again for your videos!

Solid video! I’ve been using fasting insulin for years. After enough time I simplified approach to considering optimal fasting insulin a five or less associated with a fasting glucose in the double digits (Ie “normal“) measured in milligrams per deciliter.

Actually never heard of TYG! I’m looking forward to checking that one out.

Have to correct Dr. Gil on the abdominal (a term I prefer over waist, which is often confused with hip to the public) circumference thresholds.

The threshold of 40 inches is only in men and only in what many consider outdated NIH criteria.

In NIH criteria the threshold for women is less than 35 inches.

Moreover, if you look at the International Diabetes Federation, they put central obesity for purposes of defining insulin resistance (a.k.a. metabolic syndrome) at a level of 37 inches for men, even lower for Asian men - and 31.5 inches for women.

Love this! Many thanks for sharing Dr Carvalho. Great that you gave several options as I am more genetically and ethnically prone to skinny diabetes as per my father. So in that context, the last method may not be applicable to me as my waistline is 31”

If you want to know if you're insulin resistant pretty quickly, take off your clothes, stand straight, and look down. Can you see your feet? If not, you have insulin resistance. The ladies might have to lift up their breasts in some cases to see, but pushing your stomach in is not allowed.

I had never heard of TYG before. However, now, when looking into it, I see that there is a great confusion on the Internet of how to really calculate it. The calculator that you reference in your video both calculates it correctly and gets the formula correct in the description. However, since then I have found calculators that calculate it differently than the formula they give for how they calculate. And then there are two different formulas used in different scientific articles. Oh, well. What a mess around such a simple formula.

Love your videos. This video should be titled - "How to estimate & guess at your insulin resistance" as there is no direct measurement, no specific standard, ...no one specific pass / fail. IR not directly measurable.

Dr Carvalho touched only briefly on the triglycerides/HDL-C ratio as a possible estimator of insulin resistance, but he thought TyG would do a better job. As I have just undertaken a low carb diet, I have a series of readings on TyG before and after the diet, ranging from 4.39 (post diet) to 4.86 (pre-diet). I also have the corresponding triglyceride and HDL-C readings. I find that (for me anyway), TyG and triglycerides/HDL-C ratio are well correlated (r=0.980, n=7). If the cutoff for TyG is deemed to be 4.49, then the corresponding triglycerides/HDL-C ratio is 0.6 (measurements in mmol) or 1.4 (mg/dL). The triglyceride/HDL-c ratio is more convenient for me because I would not need to go online to use the TyG calculator.

Thanks for the info!
I never knew triglycerides could be used to measure insuline resistance.
The more i learn the more confused i get whether fat consumtion, especially saturated fat (even if you don't exceed in calorie intake) can cause diabetes, because i used to think only refined carbs and sugars are responsible.
