I think it’s a two-step process. First, you do have to “manually” REMOVE the blackheads by a dermatologist or an aesthetician in order to have a clean face. Second, once removed, you can apply the retinoids to MINIMIZE occurrence.

I had really prominent blackheads all my life. They weren’t large but there were a lot of them. I have combination skin. (Oily t-zone is where the blackheads always appeared). I’ve been using Cereves retinol and I’ve had amazing results with it. However I do use it every night, put it on and let it dry, before putting my lotion on. I also use their sunscreen lotion in the morning before applying my makeup (don’t use foundation but rather tinted moisturizer). My blackheads are way fewer and less noticeable when/if I get them.

Sucks when you have sensitive skin+ eczema, I cant use retinoid/retinols or salicylic acid. Literally anything I see recommended to get rid of clogged pores. The only way I can do it is by physically removing them and that’s temporary

Hey Dr. Dray, is it safe to shave / ‘dermaplane’ the face when sebaceous filaments are present? I know it’s not recommended over active acne but does it pose the same risks when it comes to sebaceous filaments?

I couldn't resist. Like I would squeeze them for real and then use retinol. How long would it take for the retinol to get rid of that without squeezing first😮

Esthetician here. I don’t understand how retinoid can completely dissolve the blackhead (especially of the size in the picture) completely. I don’t see that happening, at-least not my experience using Tretinoin for years, and having minor blackheads, whiteheads and sebaceous filaments. I feel like oil plug build up will just linger in the pores, potentially stretching the pore unless manually extracted. I like the idea of gentle properly done extractions in conjunction with retinoids and retinols to prevent major congestion as seen here. Please explain further how retinoids alone can completely unplug those crazy black heads?

So...in the case of the pictured black heads, does the retinoid use cause the black head clogged pores to self empty?

I use it everysingle day (retinol) for 10 years and i still get black heads on my nose. A lot of them.those tiny tiny dots all over my nose. How do i get rid of them!!! Cuz retinol & scrubbing them 5-6x a week doesnt wor
