Answer for question 1, default setting is HI

Answer for question 2,
The maximum difference is:
±20 ft between both PFDs
And depending on the aircraft configuration:
±100 ft between ISIS and PFDS, or
±300 ft between mechanical standby altimeter and PFDs.

If the APU is supplying, pack controllers select HI flow automatically, independent of the selector position :)
