1. The sorrowful expression on the Blood Starved Beast emphasizes the tragedy of Old Yharnam.

2. The excessive growth of its fangs implies a decent length of time has happened since the fall of Old Yharnam and the main storyline.

3. The lack of eyes ties to the theme of insight. No eyes= no insight, a fall to beasthood.

This is my favorite boss, beast, monster (2nd only to the xenomorph). I didn't know it actually had a detailed skull and face behind. Remarkable. I always thought it would wear a furious expression, but in reality it's just starving and suffering.
