Bloodborne was a such a wild ride on release. Nobody knew what insight did or what was happening; everyone was trying to figure out the setting. So many theories as to what the hell what was happening with the mysterious light outside the Cathedral that picks you up if you're not fast enough. Imagine being the first guy to reach 40 insight or defeating Rom and then having even more questions when you see what's causing it

the Amygdala seem to be what inspired the whole eyes on the inside thing. Provost/Master Willem saw them with their eyes inside the brain, and thought that their form was the key to greater understanding.

Also remember the Amygdala is a part of the human brain (though pronounced differently) and it's responsible for processing stimuli associated with fear or danger. You know that feeling that something's watching you? That's likely caused by the amygdala, though science is unsure as to why it occurs when there is no percievable threat.

My theory is Miyazaki designed these Amygdalae to have eyes on the inside of their fleshy heads like that as inspired by the real amygdalae responding to threats we don't perceive so they must have "insight" into another realm or state of being, which plays along with the rest of theme of the game where "insight" is interchangeable with "in sight" or "eyes on the inside" that can see what is normally unperceivable

My own theory is that the boss one we fight is a dream specific Amygdala. Basically a subspecies of the ones outside the dreams. Notice that the one fallen into the Hunters Nightmare also doesn’t have face tentacles

Something else that makes my skin crawl is they’ll contort their necks to look at you. While I was in unseen village i decided to pay close attention to the Amygdalas watching me, looking at them i noticed how they won’t always track you until you’re in a field of view, like outside instead of indoors, at that point they’ll turn their heads at you even ones in uncomfortable positions. I messed with this on a specific Amygdala that was hanging upside down I think, and it would be directly looking at you, and quite literally it was breaking its neck just to track me, and I think at some point it’s head would be considered upside down, either a glitch or intentional (which would be more unsettling) since that means they can twist their heads 360 degrees in seemingly any direction, if you leave the field of view they return their heads to normal so watching this one go from a twisted head position and quickly back to its normal position made my back shiver.

Amygdala is one of my favorite bosses in bloodborne. It's always been extremely intriguing to me. I love the design of the many eyes and the weird body. Just really brings out this sort of nightmarish element present in bloodborne. Then the fact that they're really everywhere, just unseen is super cool.

No facial tentacle means less attackable parts, especially with Amygdala's design - a giant creature with its head high above the player that rarely an attack can land on its head - but with soft squeeshy tentacles dropped down it would allow the head to have a bigger hitbox thus more vulnerable to a untrained player. The other thing to mention is that obviously From can not make its legs disappear too so they just make them super strong against physical attacks, but they balanced it with a mechanism to bait the jump attack, though the idea of "legs are too strong to provide meaningful harms" would be burried in a new player's mind already. Very smart mind game, very smart design.

Fun fact: you don't really need 40 insight to see the Amygdala crawling on the buildings, all you have to do is look REALLY close into it, getting good camera angles and lighting you can see it's not completely invisible, but rather transparent.
