I worked over 10 years for a company that negotiated medical bills (called a cost containment/bill adjudication company). We would regularly be able to negotiate bills down by 90%. One example of an abusive cost: charging you for a whole box of latex gloves when only 2 gloves were used.

If it’s necessary to travel, try to get the insurance company to reimburse at least a portion of travel costs, especially air fare, hotel fees, rental car, and food. Keep meticulous records. You won’t know unless you ask!

By errors he means people trying scam. If it was an error all this time people have been paying and their books reflect they have more money than they should then they need to make it right but they don't because they are knowingly scamming
