They used to cut so fast because the patients were fully awake and had no pain relief beyond alcohol and laudanum (a solution of 10% opium taken orally, decent pain relief for the time but Jack shit when your leg I being sawed off). It was literally people's job to hold the patient down while the limb was being cut off. Liston apart from this surgery actually had a better survival rate than most both because of his speed and because he generally washed his hands and implements lol, though he did have a bad habit of holding his knife in his teeth. His paper on cleanliness in obstetrics (pregnancy and birth) outraged surgeons as "cleanliness is next to prudishnes in surgery".

Fun fact, there is actually no record of this surgery and it’s probably a myth spread by one of the many surgeons who hated him.

Liston was also huge advocate for anaesthesia when it was first discovered

We had a young General Surgeon come to join our hospital and he bragged he could do an Appy skin-to-skin in 7 minutes. A few of the more senior General Surgeons were not very impressed, and expressed concern amongst each other that if he had a bleeder or other issue, they were “not going to bail him out”. The Senior Anesthesiologist basically told them to calm down, he would take his cases and make sure things went well. He did 7min, anesthesia was quick, patient had uneventful recovery and sent to the floor for the evening. No one died.

Funny thing is, this is the same exact doctor that invented hand washing, popularized ether (the first anesthetic), he was one of the first doctors who didn’t treat his patients like shit, and he was the first doctor to realize that dirty equipment led to infection

Aside from this incident, he was actually a decent doctor for the time, and he sterilised his instruments and clothes in a time when germs weren’t known about and having gunky aprons and instruments was almost like a trophy for surgeons. They literally used to cut into people with other people’s dried fluids still on the knife. This guy cleaned his with alcohol, and just from that, the mortality rate of his patients was significantly lower than those of other surgeons.

The reason he had to work so fast was that there was little in the way of anaesthesia, and stopping bleeding was most likely harder too. Time really was of the essence.

If the patient consented. I still support the idea that members of the public should be able to view surgeries and staged autopsies. Not only can this raise funds for the hospital but also educate members of the public.
