Turns out if you reject her, you do not gain the ability to level up. Also I realized after recording that there was a "trade" option in the bonfire/grace menu so you can talk to her again whenever

I played like 5 hours of the game maidenless, defeated a couple of bosses and just now I realized that you need a Maiden to level up and progress

You can now level up at any "bonfire" and there is still a "Level up Lady"

I'd assume that the cancel option is for the hardcore difficulty, where you can't level up your stats and just do it all naked.

I hate how you’re basically forced to having a maiden or else you miss out on leveling up, just imagine someone beating the game and finishing the story without maiden. I don’t even know if they have an ending where the maiden is not there.

I guess every SL1 challenge ever is gonna take this option, unless she appears again at some point where you're forced to have a maiden even if you don't need one. Still, I have to wonder how it will impact the story if you reject her.
