Pain in the joints is one of the most common problems that we come across these days. There are many reasons that affects the health of the joint. In ayurveda this joint pain can be categorised into various sections depending on the kind of complaint or the attributing factor towards it. It could be the traumatic or degenerated joint lesions or inflammatory joint lesions. Trauma can be to the joints, the soft tissue, the tendons, the ligaments or the menisci and various other tissues related to the joint. The trauma leading to the injuries and the common complaint is pain in the joints. The degenerative issues usually seen in old age. It is because of continuous usage of these joints. There could be wear and tear changes leading to sandhigadh or vadha or what we call as osteoarthritic changes. Here there is complete destruction of the soft tissues leading to bone to bone rubbing and the symptoms like pain, stiffness and such things arises. Next is inflammatory joint lesions. This could be due to the over load of toxic what we categorise in ayurveda as amavadha. This amavadha is nothing but accumulation of toxic systemic. The management is usually based on which category the joint pain belongs to. Is it under traumatic, degenerative or the inflammatory varieties. The first instant of any trauma is rest and whatever the traumatic result is based on that the joints have to be treated. Either the oil application or the lepams we categorise as based on the presentation of the condition. In degenerative joint lesions what we call as santhigadh or vadha initially oral medicines are given to arrest the progress of these degeneration and further management and the rejuvenation of the joint is the primary goal. There the external therapy also plays a very important role. It is the oil based therapy. It could be the abnangam, svethanam and various application of lepams. The spiritual treatment followed by the laboratory is one of the primary treatments of ayurveda aimed at arresting the degeneration and also rejuvenating the joints. When it comes to inflammatory joints lesions systemically if the conditions are very acute then systemic medicine has to be given to clear this amavastha or the toxic material has to be cleared out using oral medication and if the condition is chronic then we need to give certain medications and flush it out. There comes the ayurvedic procedures called the panchakarma treatment which is the purificatory treatment. After this initial cleansing the next step is external therapy. It can include the oil based treatments and the oil less treatments also. Oil less treatments include the choornasvedhanam and the application of various lepas, the dhanyalatharaas. These are done to first clear of the condition and then oil based tretamnets are given to improve the mobility or remove the stiffness of the joint. So based on the conditions or the reasons for the joint pains various medications orally is also given to the patient. It can range from the maharasnadhi kashayam in case of the sandhigadha vadha or the rasyaerandadhi kashayam and amruthothara kashayam in case of inflammatory joint lesions with gulgulathikthakashayam, yogarajagulgulu, kaichoragulgulu, various other gulgulu preparations are also given. This is how following ayurvedic procedure a joint pain or joint lesions are usually treated. Along with that the patient’s immunity also has to be improved up on.
