Diomedes I believe gets killed like this if you kill a bunch of Blood Ravens in the level, but also kill Galen so that he can't corrupt Diomedes. Otherwise, he either lets you go, or is approached by Galen.

This occurs BEFORE the revelation of the traitor, so Avitus is always available to deliver this execution; including, I believe, if he is not taken on the mission.

It's not always Avitus it depends on which squad member has the highest corruption rating and if they are all 100% pure the traitor will be the Techmarine
